Dementia Starts Decades Early — Chapter One | Notes on Aging Well
Chapter One · Brain Health
July 22, 2026 · 4 min read
Dementia doesn't start with memory loss. It starts up to 20 years earlier.
While you're still busy skipping sleep, ignoring your blood pressure, and telling yourself you'll "deal with health" after 50.
Here's the fact that stopped me: brain changes linked to Alzheimer's and dementia can begin quietly in your 30s and 40s, with zero visible symptoms. By the time forgetfulness shows up, the disease has usually had a 15–20 year head start.
15–20 years is the typical head start brain changes get before symptoms ever appear.
And in India, this isn't a someday problem. Alzheimer's already accounts for 70–75% of all dementia cases here. Our 60+ population is set to jump from 138 million to nearly 194 million by 2031. We are not ready for what that means.
What's actually driving the risk, according to research:
→ Unmanaged blood pressure and blood sugar
→ Chronic sleep loss
→ Physical inactivity
→ Smoking and heavy drinking
→ Social isolation
→ Untreated hearing loss
None of these sound like "aging" problems at 35. All of them compound by 65.
I started noticing this pattern through the families I meet in our adaptive care work — people caring for a parent through dementia, always saying some version of: "I wish we had known sooner."
We can't control genetics or age. But a meaningful share of dementia risk comes from things we control every single day — decades before it would ever cross our mind.
This is Chapter One of a series where I'm breaking down what actually helps — not wellness-industry noise, but what the research says. Because dignity in aging starts at 30, not 70.
More next Monday.
